Back after 11 years
I came across my blog suddenly, after 11 years.
Was pleasantly surprised by what i saw. Seems like the younger me was a much more optimistic soul.
Life update: I was in MBBS when i started this blog. Now, i have finished my MS, MRCS and MCh and i now specialize in Urology. Presently, I'm working as an Assistant professor in Rajiv Gandhi Super Specialty Hospital (OPEC).
what made me come back to blogging?
So today (23/06/25), i operated on a 9 year old with PUJO (pelviureteric junction obstruction). i was a little anxious because i had not operated on a child before. And, a senior consultant of mine, wanted me to approach through a different incision which i was not used to, making the orientation difficult. Anyway, i proceeded with the surgery, followed the steps i was supposed to and reached the kidney, identified the ureter, looped it and dissected it upwards. To my surprise, i couldn't find the point of narrowing, which is usually at the level of the lower pole. So i went further above, only to reach the hilum without seeing any transition. The child was previously stented, leading me to assume the obstruction had disappeared because of it?. But i didn't want to base a decision made on an assumption, hence i called my senior consultant. He dissected it further towards the hilum, only to realise it was an intrarenal pelvis with the PUJO very close to the Renal Parenchyma. Thankfully, my Senior is trained from CMC vellore, a very reputed college for urology, so it was easy for him to make a decision. we initially thought of doing it like a heinicke-mickulicz procedure, which would mean no excision of the redundant part, but went ahead with Anderson Hynes Pyeloplasty, which made the suturing very difficult because of the short end of pelvis we had. Sir did the posterior layer and let me close the anterior layer and put a drain and close. I was kinda glad i got to see such a case and wanted to pen it down somewhere. Hence the post.
There have been many days in the past few years that i have seen interesting cases. But it never occured to me to have it written.
Maybe today is the change. Hopefully this will become a Routine.
